Vendor Account Setup/Change Request Form

This form along with several other items will need to be completed and returned in order for the Finance Department to have accurate information regarding your company’s accounts payable with the City of Woodway. Since the City of Woodway is tax exempt, once the Finance Department receives your completed packet we will send a Tax Exemption Certificate to the person listed as the A/R contact for your records.

The Internal Revenue Service (IRS) requires the City to have your taxpayer identification number (TIN) and certification on file. Under Federal Regulation Section 6109, your company is required to provide the City with this information on the W-9 form included with this packet. If you fail to furnish the City with this information, your company may be subject to a fifty-dollar ($50) penalty imposed by the IRS under section 6723 and all payments made to your company could be subject to 28% backup withholding.

The City will not have to file an annual information return, Form 1099 Misc., for your company if it is considered a corporation, tax-exempt organization, government agency, or other exempt payee. However, law does require us to have your TIN and payee type on file. If you do business as an individual or sole proprietor your social security number will serve as your TIN.

The selected contractor or vendor is required to complete the below. Certification is required by Texas government code section 22770.001. By signing, the company hereby certifies the following:
  1. The company does not boycott Israel; and
  2. The company will not boycott Israel during the term of the contract.
The following definitions apply to this state statute:
(1) "Boycott Israel" means refusing to deal with, terminating business activities with, or otherwise taking any action that is intended to penalize, inflict economic harm on, or limit commercial relations specifically with Israel, or with a person or entity doing business in Israel or in an Israeli-controlled territory, but does not include an action made for ordinary business purposes; and
(2) "Company" means a for-profit sole proprietorship, organization, association, corporation, partnership, joint venture, limited partnership, limited liability partnership, or limited liability company, including a wholly owned subsidiary, majority owned
subsidiary, parent company, or affiliate of those entities or business associations that exists to make a profit.
State law requires certification from a Company for contracts (which includes contracts formed through purchase orders) involving goods or services regardless of the amount.

This exempts you from the Certification is required by Texas government code section 22770.001.

Senate Bill 19 Verification

I, the undersigned representative of this company/business (hereafter referred to as company), being an adult over the age of eighteen (18) years of age, verify that the company named-below, under the provisions of Subtitle F, Title 10, Government Code Chapter 2274:
  1. does not have a practice, policy, guidance, or directive that discriminates against a firearm entity or firearm trade association; and,
  2. will not discriminate during the term of the contract against a firearm entity or firearm trade association.
Pursuant to Section 2274.001, Texas Government Code:
  1. "Ammunition" means a loaded cartridge case, primer, bullet, or propellant powder with or without a projectile.
  2. "Company" means a for-profit organization, association, corporation, partnership, joint venture, limited partnership, limited liability partnership, or limited liability company, including a wholly owned subsidiary, majority-owned subsidiary, parent company, or affiliate of those entities or associations that exists to make a profit. The term does not include a sole proprietorship.
  3. "Discriminate against a firearm entity or firearm trade association":
  4.  means, with respect to the entity or association, to:
  5.  refuse to engage in the trade of any goods or services with the entity or association based solely on its status as a firearm entity or firearm trade association;
  6. refrain from continuing an existing business relationship with the entity or association based solely on its status as a firearm entity or firearm trade association; or
  7. terminate an existing business relationship with the entity or association based solely on its status as a firearm entity or firearm trade association; and
  8. does not include:
  9. the established policies of a merchant, retail seller, or platform that restrict or prohibit the listing or selling of ammunition, firearms, or firearm accessories; and
  10. a company 's refusal to engage in the trade of any goods or services, decision to refrain from continuing an existing business relationship, or decision to terminate an existing business relationship:
  11.  to comply with federal, state, or local law, policy, or regulations or a directive by a regulatory agency; or
  12. for any traditional business reason that is specific to the customer or potential customer and not based solely on an entity 's or association 's status as a firearm entity or firearm trade association.
  13. "Firearm" means a weapon that expels a projectile by the action of explosive or expanding gases.
  14. "Firearm accessory" means a device specifically designed or adapted to enable an individual to wear, carry, store, or mount a firearm on the individual or on a conveyance and an item used in conjunction with or mounted on a firearm that is not essential to the basic function of the firearm. The term includes a detachable firearm magazine.

Senate Bill 13 Verification

I, the undersigned representative of this company/business (hereafter referred
to as company), being an adult over the age of eighteen (18) years of age, verify that the company named-above, under the provisions of Subtitle F, Title 10, Chapter 809, Government Code 2274:
  1. does not boycott energy companies and;
  2. will not boycott energy companies during the term of the contract.
Pursuant to Section 2274.001, Texas Government Code:
  1. “Boycott energy company” has the meaning assigned by Section 809.001; and
  2. “Company” has the meaning assigned by Section 809.001, except that the term does not include a sole proprietorship.
